Suggested Subject: Regret for My Sudden Absence

Dear Sir,

Please accept this email as official notice that I could not be present at work on 28 June 2017 due to a severe cold. Jon and Micah stood in for me when I was away. They completed all my assigned duties for that day.

I apologize that I could not communicate with you before staying off work. I am sorry if there was any inconvenience the company suffered on account of this.

I have attached the medical report to this email.
